Notes on PMAC general installation

1. A single PMAC order consist of:

    1. Turbo PMAC PCI lite 
    2. ACC-28B A-D interface
    3. ACC-8D OPT 6 optical isolated encoder inputs
    4. ACC-8D 4 channel breakout board
    5. PMAC executive CD-ROM
    6. PMAC documentation

   The tcs3 project purchased 3 pmac. The board as installed
   as indicated below:

2. All default jumper setting/options were used, except:
